Linkages to Care (LinC): Scaling Effective Models
Linkages to Care (LinC): Scaling Effective Models builds on RIZE’s original LinC grant program, which strengthened partnerships between hospitals and community-based organizations to help people with substance use disorder transition from inpatient and emergency department care to ongoing treatment, recovery, harm reduction, and social supports.
A RIZE-funded evaluation conducted by the Institute for Community Health found that LinC increased collaboration between hospitals and community-based organizations and helped more people connect to and remain engaged in care following discharge. Based on these findings, RIZE provided an additional year of funding to five organizations from the original LinC cohort to refine, document, and strengthen the models identified as particularly effective in the evaluation.
This investment will help transform promising practices into durable, replicable systems that strengthen transitions from hospital to community-based care and improve outcomes for people with substance use disorder across the Commonwealth.
